WMC San Antonio has been issued $299,339 in proposed penalties and citations alleging two willful violations after a worker died in a fall, the Labor Department said Monday.
The Occupational Safety and Health Administration found the company allegedly allowed employees to ride atop an unsecured, site-made forklift attachment to move wire mesh bundles at the plant. The worker had been transporting bundles to flat-bed trailers when the attachment slid off the forks, which caused them to fall, according to OSHA.
